El Capo está decidido a terminar con el negocio que en el pasado le dio dinero y poder, así que escribe con Terry, el proyecto OPAC. Proyecto que consiste en distribuir droga gratuitamente en las principales ciudades del mundo. Bruna y Tato, sus fieles lugartenientes, que han estado en libertad estos 3 años, echan a rodar un plan de fuga para el Capo, en el que colaboran nuevos aliados: Pitre y Kyara. Kyara filtra a Asimov, el líder de la mafia rusa en New York para conseguir beneficios para el Capo y el OPAC, aunque ella lo hace por dinero, cuando conozca a León Jaramillo se unirá a su equipo por amor. Kyara se convertirá en una más de las mujeres del Capo, y aunque quisiera ser la única, tendrá que entender que el Capo siempre amará a una sola mujer: Isabel Cristina.
En la fuga del Capo se ven vinculados, Steven Brandon, director de la DEA y el coronel Velandia, quienes aceptan sacar a León Jaramillo para ayudar a rescatar a la familia de Brandon que fue secuestrada. Con El Capo en las calles de New York se inicia el Opac y se mueve la maquinaria para conseguir la droga que será regalada; así se ven enfrentados a Asimov y Niño Malo, el hijastro de Pacífico Blanco. Sin embargo no todo es lucha para el Capo, pues la libertad es la oportunidad de ver a su familia, y reconquistar a Bruna, aunque esto último no sea posible, porque ella tiene un romance con el coronel Velandia. Una relación que Bruna no disfrutará, pues aún ama al Capo, pero no podrá recuperarlo porque cuando intente volver con él, descubrirá que está embarazada.
En un final apoteósico, El Capo se encuentra frente a frente con el criminal más poderoso de mundo. Un enfrentamiento donde sólo uno puede sobrevivir, pero las cartas están echadas, Jacob ha perdido no sólo dinero sino credibilidad en la sociedad, y está por ir a la cárcel, mientras que León Jaramillo, está protegido por la DEA y tiene la posibilidad de empezar una nueva vida con Isabel Cristina, Julieta y su nieto, con nuevas identidades y nuevos sueños.
